In requesting a Certificate of Eligibility to accept the responsibility of a Baptism Sponsor, I understand I am asking the Church to admit me to the role responsibility of a sponsor under both the guidance and wisdom of Canon 874 of the Code of Canon Law.

I have given prior witness faithfully demonstrating my capability and dedication to fulfill the life-long duties of sponsor duties in harmony with the faith and role to be undertaken.
And in asking, I too am asking my pastor to attest to the pastor of the candidate for baptism or confirmation that I am in good faith and character to faithfully represent and can fulfill together with the celebrant who baptizes or confirms the Church in receiving the candidate as a member of the Body of Christ to be received into full communion with the Catholic Church and to assist the person in Christian initiation given the functions and role a sponsor exercises.

Thus, the parish membership of the person receiving the sacrament is immaterial. An individual baptized in a time of emergency such as at a hospital would be registered at the nearest parish. In the case of a merged or no longer listed parish, contact the church to the closest church in proximity to the church of baptism.
