WHEN A DEATH OCCURS: STAFF REMINDER
Complete and submit this form immediately upon notification of death to record vital statistics and biographical information about the deceased. This information will be reviewed and confirmed with the family and used to complete the death announcement shared through social media and in preparing page one of the service program.Step 1: Share Resources With Family
As soon as practically possible, send the Bereavement Planning Guide for Families link to the deceased's primary contact by text or email.
Step 2: Complete Notice of Death Form 1 Below
The priest or deacon who was first notified of the death (by funeral home, hospital, hospice, family, or if present at death) must complete Notice of Death Form 1 immediately.
Step 3: Family Completes Biographical Information
The family will complete the Biographical Information form through the Bereavement Planning Guide.
Step 4: Meet With Family
Arrange an in-person meeting at the parish office to finalize funeral arrangements.
Step 5: Complete Notice of Death Form 2
After the funeral planning meeting, the priest, deacon, or delegated parish staff member must complete Notice of Death Form 2.
